Frontline on PBS did an extensive analysis of the collapse and came up with the same things you mentioned. Jeff Dantzler - Re: protecting 4x4 fence posts set in concrete - The guy that built the fence at the house I bought in 2001 had a good technique. He poured concete footers with an embeded chunk of structural steel angle iron sticking out one foot and plumb. The angle has 2 holes for bolts drilled in it. The fence posts simply bolt on.
