The only problem is that the slab has cracks here and there. I think it is because the foam panels were not placed over PERFECTLY flat dirt and the weight of the concrete has caused the insulation to settle over the voids. Do what you can to get the subgrade flat and then do what you can to reinforce the slab. Good...
