I bought one in advance from telestial.com It came with instructions (in English) for the set up once I was in Italy, and I was able to give the number to family before I left the US. As I recall, incoming calls were free. It came preloaded with 60 minutes of time; calls to the US were 50 cents/minute. I didn't use anywhere near the 60 minutes. At my first hotel I just swapped out my regular SIM for the Italian one, and did the set up. My phone was gsm and unlocked.
